2027 IRMAA brackets
The 2027 IRMAA brackets have not been announced. CMS sets them each autumn, normally in November, in the same notice that sets the Part B premium and deductible. Until that notice exists, nobody has the 2027 figures — including the sites that print them. This page will carry them the day they are published, and until then it will tell you what is actually known instead of guessing.
Half of your 2027 surcharge is already decided.
A surcharge has two ingredients: your income, and the thresholds it is measured against. The income for 2027 comes from the tax return you filed for 2025 — which is already filed, and cannot now change. The only unknown is where the lines fall. So if you want to know roughly where you will stand next year, look up your 2025 figure now and compare it to this year's published table; the lines move a little with inflation, but they do not move far.

Why we do not print a projection
The thresholds are indexed to inflation by statute, and the method is public: a twelve-month average of the Consumer Price Index for All Urban Consumers, compared against a base period, then rounded to the nearest $1,000. We implemented that method and back-tested it — it reproduces the announced 2024, 2025 and 2026 thresholds exactly. So the objection is not that the arithmetic is hard.
The objection is that one of the twelve months does not exist. The averaging window for these brackets runs from September 2025 to August 2026. Today 9 of the 12 months are published. Some of what is missing is simply not out yet and will arrive on a scheduled date. One of them is different.

October 2025 was never collected, and no future release will supply it.
BLS could not collect October 2025 reference-period survey data during the lapse in appropriations that ran October 1 through November 12, 2025. Rather than collect retroactively — which BLS judged costly and potentially inaccurate, since its price surveys do not rely on respondent recall — it imputed October prices by carry-forward and published no October 2025 index for most series, including this one. The BLS Public Data API returns a dash for this month with footnote X, 'Data unavailable due to the 2025 lapse in appropriations'. This month is permanently absent: unlike an unpublished month, no scheduled release will supply it.
That is why this page carries no projected table. Averaging the eleven months that do exist is not the published method with one input missing — it is a different method, and the difference lands on a bracket edge, where being wrong by one step costs a reader an entire tier for a year. What CMS will do about the gap is a decision for CMS, and we will report it rather than pre-empt it.
What to do with the autumn
The 2027 thresholds are fixed by then and nothing you do now changes them. What is still open is the income that will set your 2028 premium, because that is this year's return, and this year is not over. If a withdrawal, a property sale or a conversion is under consideration, the distance between your income so far and the next published threshold is the figure to look at while it can still be acted on.
